Standing outside Greg’s cottage, with the door closed, Phil Jenkins cleared his throat.

“Out with it, Phil. You think I’m pushing this and he is risking the loss of his respiratory drive,” Sister said.

“There is that risk with titrating his O2 to that extent,” Phil said. “Especially if he’s sleeping.”

“He’s dying, Phil. The cancer is profoundly aggressive. I’m trying to give him a few more good hours with his children.”

“It’s a good goal, Sister,” he said on a sigh. “But you are going to have to monitor him real close to make sure he doesn’t a) lose his respiratory drive or b) go into CO2 narcosis. He’s here for palliative care, so that pretty much rules out putting him on ventilator to correct things if he goes into narcosis.”

“I know. But narcosis is easier to correct than death is.”

“Okay. We don’t know one another well. I’m covering my own ass by having this discussion with you. We both know that. But you are the doctor, and it’s my duty to make sure you know the ramifications of your orders,” Phil said. “Still the final call in patient care is yours, and the responsibility, also.”

“I appreciate your position, Phil,” Sister Elizabeth said. “I know just how closely I will have to monitor him, and I know the potential side effects of giving him that much oxygen.”

“I have no doubt that you are a good doctor. I heard all about how well you handled Mrs. Gilbert. My wife is one of the nurses on the Stroke Team. I just worry about giving anyone this much oxygen.”

“So, do I. But, I need to give him time with his children, if I can.”

Phil nodded and smiled. “It is a good goal. I’ll bring over another couple of tanks.”

“Thank you, Phil.”

“Just be careful. Make sure his kids and Greg know the dangers of this, and get them to consent. Cover your own derriere, Sister. Please?”

“I will.”

Phil walked away.

She stood there for a long time, looking at the sky and praying. She was about to go back in when James came out carrying a basket.

“He still awake?”

“Yep, and laughing with his daughter,” James replied. “He was hungry so I cut him half of a roast lamb sandwich. And I’ll make dinner for them again tomorrow night. They’ll come to brunch tomorrow.”

“Don’t plan too far in advance, James,” Sister advised.

“I’m not,” he replied. “It’s pretty extreme to give him that much oxygen, isn’t it?”

“Yes. It is.”

James nodded. “I’m sure you know what you’re doing.”

“It’s not my first rodeo.”

“No, but it may be the first time that you had a connection to the dying person. That has to be difficult.”

“Putting it mildly.”
